AS.CSV Template

Hi ,

Can someone help in creation of AS.CSV template for the below two tables :-
CAPACITY_COMMITMENT
PROCESS_CONTROL_REQUEST.

It will be really helpful. If someone can guide how to create the AS.CSV template for other tables ?
I tried to access the AS.CSV servlet , but it was not accessible.
